How you’ll make an impact in this role

Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

• Lead the implementation and continuous monitoring of personalized patient care plans to drive successful recovery and optimal outcomes.
• Coordinate as the central point of contact for all clinical disciplines, ensuring a holistic, seamless, and patient-centered experience.
• Execute physician and nursing orders promptly and precisely, delivering critical prescribed treatments and maintaining continuity of care.
• Monitor and accurately document the patient's condition and progress, communicating all critical updates to the interdisciplinary team for timely intervention.
• Develop and finalize comprehensive discharge plans, empowering patients and their families for a smooth and safe transition back to their community.
  
  

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
  
  

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
